Services include adult and pediatric care; Medicaid, Medicare, and insurance accepted. Dads can call to schedule intake; bring photo ID, insurance card, proof of income, and any prior medical records. Services available regardless of ability to pay. Office open Mon-Fri 8am-5pm.
